The plot is not complicated, a handsome guy with depression went a little cuckoo and talks to a freak in the form of a cute (first love) girl.
There is not much choice here, only whether you will add this or that girl to your harem. There is incest, I would even say a lot, mom, stepsister, daughter with mom (like everything).
